Oak Trace community received Telligen’s Blue Ribbon in COVID-19 Vigilance Award. This award recognizes their commitment to preventing the spread of COVID-19 and places the organization on Telligen’s Illinois 2022 Blue Ribbon in COVID-19 Vigilance Recognition List.
“Oak Trace team members and residents came together to control the spread of COVID-19 in our community,” said Andy Kazmierczak, executive director of Oak Trace. “With vaccination rates of 97% for our team members and 99% for our residents, it was a team effort to protect the health and safety of everyone in our community.”

It is our commitment to ongoing training and support, quality of programs, and our dedicated and highly skilled front-line caregiving team members that allows us to accomplish amazing outcomes.”

Nursing homes on the recognition list have met criteria to prevent the spread of COVID-19 for the safety of their residents and staff. This includes completing a COVID-19 preparedness attestation and achieving a staff COVID-19 vaccination rate of 85% or greater, and a resident COVID-19 vaccination rate of 95% or greater.
